FEES AND INSURANCE
We believe that access to mental health services should be available to all; therefore at Stephens Behavior Consulting, LLC we provide several options. In order to best access your insurance benefits it is important that you contact your insurance provider to verify benefits (deductibles/co payments/co insurance/ in network versus out of network-specific to the provider). We will also do everything we can to help you in this process. Since we have multiple providers it is important to verify that the provider you will be seeing is the one you are checking benefits for, as some may not be in network with your specific insurance company. If your specific insurance company is not listed below we may still be able to accept your insurance; please check with our office and your insurance to verify prior to your initial appointment.
Payments:
All co-pays, deductibles, and co-insurance are due at the time of your appointment. Any outstanding balances will require a current payment plan in effect in order to continue services.

PRIVATE PAY
Private Pay: If you do not have insurance coverage for mental health benefits at this time or choose to opt out of use of your insurance benefits the following would be used as our private/self pay rates; these are not eligible for OON benefits or superbill if your provider is INN with the insurance company-
Self Pay Fees: (Fully Licensed/Provisional Licensure under supervision)
Initial Assessment/Intake:: $175/$150
Individual/Family 50 minute sessions: $150/$125
Mental Health Group Session: $45/group
Support Groups: Free
ABA Private Pay Rates:
Initial Assessment/Plan Development/Initial Plan review: $800.00
Plan Implementation by a technician: $60/hour
Plan Implementation/Program Development/Training by BCBA: $125/hour
ABA Group Therapy: $45/hour
*Sliding scale may be available based on income and need, must supply proof of income
